Congestive heart failure
Adverse Reaction:
Cardiovascular - hypotension 10% symptomatic hypotension 4% asymptomatic hypotension 7%
ventricular tachycardia 4% non sustained VT 4%
ventricular extrasystole 1% angina pectoris 2%
CNS - insomina 3% dizziness 2% anxiety 3%
GI - nausea 5% vomiting 2%
Miscellaneous- headache 20% abdominal pain 4% back pain 3%
Contra-Indications:
Hypersensitivity to the components
Special precautions- Low cardiac filling pressure- admin of nesiritide should be avoided in
patients suspected of having known to have low cardiac filling pressures
Cardiovascular - nesitiride nay cause hypotension
Renal effects- nesiritide may affect renal function in susceptible individuals .
In patients with severe heart failure whose renal function may depend on the activity of
renin-angiotensin- aldosterone system treatment with nesiritide may be associated with azotemia
Hypersensitivity reactions- no serious allergic or anaphylactic reactions have been reported with
nesitiride
Special risk- nesiritide is not recommended for patients for whom vasodilating agents are not
appropiate, such as patients with significant valvular stenosis
Pregnancy - use during pregnancy only if the potential benefit justifies any possible risk to fetus.
Lactation- caution should be excercised when nesiritide is administered to a breast-feeding woman
Children - safety and efficacy of nesitride in children have not been established
Dosages/ Overdosage Etc:
Indication-
Congestive heart failure
Dosage-
Recommended dose of nesiritide is an IV bolus of 2mcg/kg followed by a continuous infusion
at a dose of 0.01mcg/kg/min .
Nesiritide should not initiated at a dose that is above the recommended dose
Pregnancy and lactation:
Pregnancy -
Use during pregnancy only if the potential benefit justifies any possible risk to fetus.
Lactation-
Caution should be excercised when nesiritide is administered to a brest-feeding woman
Children -
Safety and efficacy of nesiritide in children have not been established
