Adverse Reaction:
Agranulocytosis, shock, intolerance (skin, bronchial tract etc) hypersens (skin etc). drop in BP on rapid injection.
Contra-Indications:
Hypersens,collapse states,hepatic porphyia or congenital G6PD defciency, in infants less than 3 mths or weighing less than 5kg.
Special Precautions:
Pregnancy, disorders of haematopoiesis.
Rarely agranulocytosis. Rarely sudden fall in BP after parenteral admin.

Dosage-
Due to very short half-life, somastatin must be given by bolus(250mcg) or by continous infusion preferably in an intensive care unit with the patient monitored for electrolytes and blood sugar levels every 4-6 hrs.
The recommended dose is 3-5mcg/kg/hr given as a continous infusion (250mcg/hr or 6mg/24hr)

Pharmacology:
Somastatin acts through somatostatin receptors. Because stomatostatin receptors are fairly widely distributed throughout the body, it is not surprising that somatostatin has many biological actions to inhibition of growth hormone release. These include inhibition of secretion TSH and prolactin by thyrotrophs and lactothrophs respy; inhibition of secretion of glucagon and insulin by the endocrine pancreas; inhibition of most gastrointestinal secretions like gastric acid, gastrin, cholecystokinin and pancreozymin.
Pharmacokinetics:
At the usual threrapeutic dose of 250mcg/h plasma range between 300pg and 3000pg/ml of stomatostatin. The half-life of somatostatin in healthy volunteers is documented to be between 1.1 & 3min. Metabolic clearance rate is about 2000ml/min.
