Interacting drugs- summary
+ Mumps Vaccine
Immunosuppressants
administration of mumps vaccine to patients receiving immunosuppressants
including corticosteroids or radiation therapy, result in insufficient
response to imuninization. They may remain susceptible
despite immunization.
Immune globulins
to avoid inactivating attenuated virus, give the vaccine at least 14 to 30
days before or 6 to 8 weeks after immune globulin. Alternately, check
anitbody titers or repeat the vaccine 3 months after 1gG
Interferon
concurrrent use inhibit antibody response to the vaccine.
Virus vacine, others
to avoid the hypothetical concern over antigenic competetion, give
mumps vaccine after or > 1 month before other virus vaccines,
However, several vaccines may be given simultaneously at
separate injection sites (eg. DTP, OPV, MMR, Hib, Hepatitis B )
Adverse Reaction:
Burning or stinging of short duration at the injection site have occured. Occasional reactions include mild fever, mild lymphadenopathy or doarrhea.
Fever > 69.4C is uncommon. Allergic reactions at the injection site or erythema multiforme occured rarely. Very rarely encphelaitis, febrile seizures, nerve deafness, and other nervous reactions have occured.
Contra-Indications:
Pregnancy- History of hypersensitivity reaction to this vaccine or any of its components (eg eggs) concomitant immunosupprerssive therapy, patients with a history of blood dyscrasia, leukemia)
Do not vaccinate persons who are immunosuppressed in association with AIDs or other clinical manifestations of infection of HIV , cellular immnue deficiencies, and hypogammaglobulinemic and dysgammaglobulinemic states.
Nonetheless, vaccinate asymptomatic children with HIV infection.
Do not vaccinate persons with a history of anphylactoid or other immediate reactions (eg . hives, swelling of the mouth and throat, difficulty in breathing, hypotension, shock) subsequent to egg ingestion since the vaccine is propagated in cell cultures of chick embryo.
Special preacutions-
Warnings-
Allergy skin testing- do not use for dealyed hypersenstivity (anergy) skin testing. Use mumps skin test antigen, a killed viral product.
Hypersenstivity- have epinephrine 1:1000 available to control immediate allergic reactions.
Immunodeficiency- do not use in immunodeficient persons, including persons with congenital or acquired immune deficiencies, whether due to genetics, disease or drug or radiation therapy.
Contains live virus. Nonetheless, routine immunization of symptomatic and asymptomatic HIV- infected persons with MMR is recommeded.
Elderly- persons born prior to 1957 are generally considered immnue and need not be vaccinated.
Pregnancy- There is no good evidence that it causes congenital malformations in humans. Do not intentionally give attenuated mumps virus vaccine to pregnant females
Lactation- Not known if attenuated mumps virus vaccine or corresponding antibodies are excreted in breast milk.
Children- Mumps vaccine is safe and effective for children > 12 months of age. Vaccination is not recommended for childfen < 12 months of age, since remaining maternal virus neutralizing body may intefere with the immune response.
Dosages/ Overdosage Etc:
Selective immunisation against mumps
Dosage-
Vaccination- 0.5ml for both children at 15 months of age and adults. Trivalent MMR vaccine is the preferred product for most vaccinations. Inject total volume of recommeded vaccine SC into the outer aspect of the upper arm.
Missed dose-
1. If you miss a dose of this medicine, and remember it on the day it should be taken, take it as directed.
2. However, if you do not remember the mised dose until the next day, take the missed dose at that time and reschedule your every-other-day doses from then.
3. It is important that this vaccine be taken exactly as directed so that it can give you protection against Mumps
Patient Information:
Ref - USP PDI Vol II 17th Edition (1997)
1.Allergies-
Tell your doctor if you have ever had any unusual or allergic reaction to
mumps vaccine or any other form of antibiotic neomycin.
2.Pregnancy-
Tell your doctor if you are pregnant or intend to have children. There is a chance
that this medicine may cause birth defects if either the male or female is taking it
at the time of conception or it is taken during pregnancy.
Studies have not been done in either humans or animals. However, use during
pregnancy is not recommended
3. Breast-feeding-
It is known whether mumps vaccine passes into breast milk. Mothers who are taking
this medicine and who wish to breast feed should discuss this with their doctor.
4.Children-
Use is not recommended for infants upto 12 months of age,
5. Other medicines-
Tell your doctor if you are using any of the following -
Treatment with x-ray or cancer medicines - may decrease
the useful effect of mumps vaccine
6.Other medical problems-
Make sure you tell your doctor if you have any other medical problems
especially-
Immune deficiency condition - the condition may decrease the useful
effect of the vaccine or may decrease the effect of the vaccines or
increase the risk of severity of side effects
Serious symptoms with fever- the symptoms of illness may be confused
with the possible side effects of the vaccine
Tuberculosis- mumps vaccine will interfere with the results of skin test
for tuberculosis for 4 or more weeks
Pharmacology/ Pharmacokinetics:
Pharmacology-
A single dose induced an effective antibody response in about 97% of susceptible children and 93% of susceptible adults.
Interaction with Food:
