Low molecular Weight Heparins- Tinazeparin, Dalteparin, Emoxaparin Refer - LMW - Heparin
Nitroglycerin infusion may diminish reviparin effect.
May displace propranolol from protein binding sites increased risk of haemorrhage when used with aspirin Increased risk of hyperkalemia when used with ACE inhibitors or angiotensin II receptors antagonists
Serious GI bleeding may occur when used with Ketorolac.
Indication:
Prophylaxis of venous thromboembolism

Adverse Reaction:
Vomiting, constipation, epistaxis, conjuntivitis, asthma, rhinitis Bleeding from skin, mucosa. GI tract and wounds and genital tract Thrombosis, thromboembolism, consumption, coagulopathy Inj site necrosis, melena, petechiae, purpura, Allergic features like pruritus, urticaria, Nausea, headache, fever, body pain, Dyspnoea, and hypotension, transcient alopecia, Hyperkalemia, hypoaldosteronism, mdetabolic acidosis, Priapiam, severe thrombocytopenia with severe thrombolic disorders and haemorhage
Contra-Indications:
Patients at serious risk of haemorrhage including blood disorders, thrombocytopaenia, peptic ulcer disease, cerbrovascular disorders, bactrial endocarditis, severe hypertension, oseophageal varices, patients who have ungergone surgery, severe renal or hepatic impairment, IM admin. patients who have developed thrombocytopaenia with heparin patients with prosthetic heart valves
Special precautions-
Elderly women in particular , lactation children renal failure, diabetes melitus platelet count done before and after reviparin therapy monitor anti-factor Xa activity in patients with increased risk of bleeding such as elderly, renally impaired patients and patients with active bleeding
Dosages/ Overdosage Etc:
Prophylaxis of venous thromboembolism (VTE)
Dose-
Prophylaxis of VTE during moderate -risk surgical procedure 1750 anti-Xa IU 2 hr prior to surgery. Continue once daily until patient if fully mobile Prophylaxis of VTE during high -risk surgical procedure Adult- 4200 anti-Xa IU given 12 hr before surgery.
Continue once daily until patient if fully mobile for at least 14 days Thromboembolic disorders Adult- initiate treatment upon confirmation of doagnosis 35-45kg - 3500 anti- Xa IU 46-60kg - 4200 anti-Xa IU > 60kg - 6300 anti-Xa IU Doses to be given bid with oral antocoagulant Usual treatment duratio with heparin is 5-7 days
