Drug Interaction:
Reduces methotrexate toxicity.
Enhances cytotoxic and neoplastic effects of fluorouracil
Increases risk of seizures in epileptic patients treated with primidone, phenytoin, phenobarb
and succinimides
Indication:
Reducing toxicity of Folic Acid antagonists- Methotrexate
Adverse Reaction:
Allergic sensitisation , rash, pruritus, eythema, urticaria, nausea, vomiting, pyrexia
Contra-Indications:
Hypersensitivity- pernicious anemia and other megaloblastic anemias secondary to
vit B12 deficiency, intrathecal and intraventricular admin.
Special Precautions-
Undiagonised megaloblastic anemia, folate dependent , tumors, pregnancy
Monitor Ca levels in patients receiving combined 5-Fluorouracil/ folic acid treatment
To be given parentrally in the presence of GI toxicity, nausea, or vomiting
Monitor CBC , electrolyte and liver function tests before and regularly during treatment
Dosages/ Overdosage Etc:
Indication-
Reducing toxicity of Folic Acid antagonists- Methotrexate
Dosage-
Do not mix with 5-FU in same inj or infusion .
Dose equal to or more than methotrexate given usually 75mg by i.v. infusion followed by
12mg by i.m. 6 hourly in severe toxicity
Less severe cases- 6-12mg i.m. 6 hourly for 4 doses
Children- not recommended
Pregnancy and lactation:
Use contraindicated during pregnancy and lactation.
Observe caution if required to be administered
