Indication:

Peripheral (labyrinthine) vertigo and associated nausea and vomiting. Menieres disease and middle and inner ear surgery (labyrinthitis).

Adverse Reaction:

Auditory and visual hallucinations, Disoreintation, confusion, Drowsiness, overstimulation, Depression, sleep disturbances, blurred vision.

Rarely -slight dizziness, malaise, headache. Slight transcient lowering of blood pressure. Dry mouth, nausea, indigestion, heartburn (rare). Skin rash, mild jaundice.

Contra-Indications:

Hypersensitivity to diphenidol, anuria (approx 90% of the drug is excreted in the urine, accumulation could occur); nausea and vomiting of pregnancy.

Dosages/ Overdosage Etc:

Indications:

Peripheral (labyrinthine) vertigo and associated nausea and vomiting. Menieres disease and middle and inner ear surgery (labyrinthitis).

Dosage:

Adults- Vertigo or nausea and vomiting.

The usual dose is 25mg every 4 hours. Some patients may require 50mg. Children- For nausea and vomiting only. Usual dose is 0.4mg/lb (0.88mg/kg).

For children 50 to 100lbs give 25mg. Do not give more often than every 4 hrs. If symptoms persist after first dose, repeat after 1 hr. Thereafter give every 4 hrs, as needed. Do not exeed 2.5mg/lb(5.5mg/kg) in 24 hrs.

1.Allergies-

Tell your doctor if you are have ever had any unusual or allergic reaction to

aminophylline, caffeine, dyphylline, oxtriphylline, theobromine or theophylline.

Also tell your healthcare care professional if you are allergic to any other

substances such as foods. preservatives or dyes.

2.Pregnancy-

Dyphylline has not been studied in pregnant women. Before taking this medicine,

make sure your doctor knows if you are pregnant or if you may become pregnant

3. Breast-feeding-

Dyphylline passes into breast milk. This medicine has not been reported to cause

problems in nursing babies.

4.Children-

Use of other bronchodilator is preferred.

5.Older adults-

As in younger patients use of other bronchodilator medicines is preferred.

Also older patients with kidney disease may require a lower dose of dyphylline,

than do older adults without kidney disease

6. Other medicines-

Although certain medicines should not be used together at all, in other cases two

different medicines may be used together even if an interaction might occur.

In such cases your doctor may want to change the dose, or other precautions

may be necessary.

Tell your doctor if you are using any of the following-

Betablockers including ophthalmics( acebutolol, betaxolol, bisoprolol, etc

These medicines may prevent dyphylline from working properly

Probenecid - this medicine may increase effects of dyphylline

Xanthine derivatives- ( aminophylline, Oxytriphylline etc. - the chance of side

effects may be increased

7. Other medical problems-

The presence of other medical problems may affect the use of dyphylline..

Make sure you tell your doctor if you have any other medical problems

especially-

Heart or blood vessel disease or

stomach ulcer or other stomach problems- dyphylline may make the

condition worse.

Heart failure or

Kidney disease- effects of dyphylline may be increased

Pharmacology/ Pharmacokinetics:

Pharmacology:

Diphenidol exerts a specific antivertigo effect on the vestibular apparatus to control vertigo, and inhibits the chemoreceptor trigger zone(CTZ) to control nausea and vomiting.

Pregnancy and lactation:

Pregnancy:

Safety for use during pregnancy has not been established. Use only when clearly needed Do not use diphenidol for nausea and vomiting of pregnancy.

Lactation:

Safety for use is not established. Weigh benefits against hazards.
