Adverse Reaction:
Local reaction s at injection site , thrombophebitis, rash, fever, rigor, bronchospasm, Anaphylaxis, GI upset Angioedema, exoliative dermatitis Arythrema multiforme.
Dizziness, mild hearing loss, vestibular disorders, Tinnitus, headache,
Blood dyscrasias Increase in serum transminase
Alkaline phosphatase and creatinine Renal failure, superinfections
Contra-Indications:
Hypersensitivity to vancomycin Prolonged use ( monitor renal, hepatic, haematological & auditory tests )
Pregnancy Lactation
Elderly
Renal impairment, superinfection
Dosages/ Overdosage Etc:
Indication-
Serious Gram -positive infections.
Serious staphylococci infections in patients sensitive or unresponsive to penicilins or cephalosporins
Dosage-
Moderate infections - 400mg i.v. or i.m. as a single dose on first day and then 200mg i.v. or i.m. daily
Severe infections - 400mg i.v. every 12 hours for 3 doses then 400mg i.v. or i.m. daily. over 85 kg body weight 3-6mg /kg
Reduce dose from fourth day in renal insufficiency
Prophylaxis in orthopedic surgery- 400mg i.v. as a sngle dose at induction of anesthesia
Children - neonates 16mg/kg body weight as a single dose i.v. infusion over 30 minutes on first day then 8mg/kg body weight as a single dose
Pregnancy and lactation:
Observe caution Contraindicated for use during pregnancy and lactation
