Adverse Reaction:
Major side effects of oral iron are gastrointestinal disturbances that include nausea, and vomiting, constipation and occasionally diarrhea Prolonged iron therapy should be reserved for patients with continuing blood loss or menorrhagia or for women with repeated pregnancies Infants and children tolerate iron preparations better than adults. Allergic sensitization has been reported following folic acid administration.
Contra-Indications:
Iron compounds are contraindicated in patients with primary or secondary iron overload, including heriditary hemochromatosis, thalassemia major , and transfusion hemosiderosis Hypersensitivity to folic acid Special precautions: Prophylactic side effects of iron should be restricted to those indiviuals at especially high risks for development of iron deficiency anemia- pregnant and lactating women,low birth weight or preterm infants, infants fed on unsupplemented formulas, growing children prone to iron prophylaxia, adults with known chronic blood loss(menorrhagia) or heriditary hemorrhagic telangiectasia Iron supplementation to patients with chronic inflammation, chronic renal failure or hemolytic anemia may cause secondary siderosis. Those receiving blood transfusions regularly should not receive iron as each mililiter of transfused packed red blood cels contains approximately 1mg of iron.

Polynuclear ferric hydroxide forms a complex with apoferrin, a protein ligand( in liver) and is stored partly as ferretin, depot iron. With time, ferritin is engulfed by lysozymes and catbolized to hemisiderin, a non specific mixture of partially degraded protein, lipid and iron. Iron is transported in the blood plasma bound to a beta-globulin, transferrin. A pink coloured complex is formed following the reaction of one molecule of transferrin with two atoms of iron per protein molecule, with a formation of a pink coloured complex. Transferrin acts as the carrier of iron through the body hence playing a major role in the iron metabolism. Als, it plays a vital role of participitating in the bodys defence mechanism against infection.
