Tamsulosin does not interact with antihypertensives like atenolol, enalapril, nifedipine, with cimetidine and warfarin

Benign prostrate hyperplasia

Most common adverse adverse effects with transulosin are headaches, asthenia, back pain, decreased libido, insomnia,rhinitis, abnormal ejaculation, and pharynigitis. Symptomatic orthostatic hypotension, is reportedin 0.2% patients receiving tamsulosin which is less than older nonselective alpha blockers.

Finasteride is assicoated with impotence, decreased libido decreased volume of ejaculate and testicular pain.

Special precautions-

Patients should be evaluated to rule out prostrate cancer and other conditions sush as infections, stricture disease,hypotonic bladder or other neurogenic disorders that might mimic BPH before starting tamsulosin/finasteride therapy Women should not handle crushed or broken tablet when they are pregnant or potentially be pregnant because of the possibility of absorption of finasteride and the subsequent potential risk to the male fetus.

Concomittant administration with other alpha blockers may increase risk of hypotension

Dosage-

One tablet per day. It should be administered half an hour following the same meal each day.

Women should not handle crushed or broken tablet when they are pregnant or potentially be pregnant because of the possibility of absorption of finasteride and the subsequent potential risk to the male fetus.

Pharmacology-

Tamsulosin an alpha 1 adrenoceptor blocking agent,exhibits selectively for alpha 1 receptors in the human prostrate.

There are two types od adrenergic receptors , alpha 1 and alpha 2. Alpha 1 receptors are associated with smooth muscle concentration in the prostrate Tamsulosin is uroselective and exerts its effects primarily on the lower urinary tract. It selectively reduces resistence along the bladder outlet without affecting bladder contration. The relaxation of prostrate and smooth muscles results in improvement in maximum urine flow and reduction in lower urinary symptoms. Combination therapy offers one more advantage in addition to complimentary mechanisms.Research group suggest that long-term combination therapy with alpha blocker and finasteride is safe and reduces the risk of overal clinical progression of BPH (benign prostrate hyperplasia) significantly more than than treatment with either drug alone. Combination therapy and finasteride alone are also found to reduce the long terem risk of acute urinary retention and the need for invasive therapy. Based on the results the American urilogical Association (AUA) has included combination drug therapy for selected patients with BPH in first ever guidelines for BPH.

Pharmacokinetics-

Absorption of tramsulosin is complete > 90% following oral administration under fasting conditions Tamsulosin is widely distributed to most tissues extensively and bound to human plasm proteins (94 to 99%). It is extensively metabolisaed by cytochrome P450 enzyme in the liver.
