Anorexia, taste disturbances,syncope,asthenia,raised intracranial pressure,
myocardial infarction, also reported
Contra-Indications:
Caution-
severe diarrhoea, toxic psychosis, CNS depression
Contraindication-
Delayed gastric emptying, phaecochromocytoma
Hepatic impairment-
Opioid analgesics are increased and prolonged and there is an increased
cerbral sensitivity when patients with renal impairment are treated with opioid
analgesics, avoid use or reduce dose.
Renal impairment-
The effects of opioid analgesia are increased and prolonged and there
increased sensitivity when patients with renal impairment are treated with
opioid analgesics, avoid or reduce dose
Dosages/ Overdosage Etc:
Indication-
Acute pain
Dosage-
Acute pain by subcutaneous or intramuscular injection
5mg repeated every 4 hours if necessary (upto 10mg for heavier
well-muscled patients
by slow intravenous injection,
quarter to half corresponding intramuscular dose
Myocardial infarction, by slow intravenous injection
(1-2mg per minute), 5mg followed by a further 2.5 -5mg
if necessary
Elderly or frail patients , reduce dose by half.
Acute pulmonary edema- byslow intravenous injection,
(1mg/minute) 2.5 -5mg
Chronic pain, by subcutaneous or intramuscular injection
Adult- not currently treated with a strong opiod analgesic
initially 2.5-5mg every 4 hoursadjusted according to response
Pregnancy and lactation:
Pregnancy-
Respiratory depressionand withdrawal symtoms can occur in the neonate if
opioid analgesics are used during pregnancy
Also gastric stasis and inhlation pneumonia has been reported in the mother
if opioid analgesics are used during labour
Breast feeding
Avoid- - although usually too small to be harmful, mothers vary
considerably in their capacity to metabolise codeine- risk of morphine overdose
in infant
