Interactions -summary

Cephalosprins +

 Heparin

  several parentral cephalosporins have caused coagulopathies: this might be additive with heparin,increasing the risk of bleeding

Concurrent use of cephazoline and frusemide increases risk of nephrotoxicity. Potentiates nephrotoxicity produced by aminoglycosides.

Prothombin time prolonged by cephazoline and caution necessary if used with anticoagulants.

Probenecid blocks tubular secretion of cephazolin thereby prolonging half-life.

Anaphylactic reaction.

Pseudomembranous colitis, superinfection, anaphylaxis, diarrhea G.I.upset, rash, blood dyscarsias. False positive reaction for urinary glucose.

Development of positive coombs test interferes with cross matching of blood.

False positive precipitation tests for proteins.

Known hypersens to cephalosporins. Safety for use in pregnancy or in premature infants not established.

Special precautions:

Hypersens to penicillins. In beta-haemolytic streptococcal infections continue therapy for atleast 10 days.

Concurrent admn of diuretics impaired renal function.

Pregnancy.

A first generation drug. :

Indication-

Respiratory tract infections, Genito-urinary tract infections, skin structure infection, Biliary tract infections, Bone & joint infection, pre-operative prophylaxis.

Dosage:

Moderate to severe infections - 500mg to 1g every 12 hours.

Severe life threatening infections 1 to 1.5g every 6 hours.

Children: Mild to moderate - total daily dose 25 to 50mg/kg/day(app 10 to 20mg/lb in 3 or 4 equal doses.

Severe infections- Total daily dose may be increased to 100mg/kg(45mg/lb)

Storage: Reconstituted Cefazolin sod is stable for 24 hours at room temp, 96 hours refrigerated (5C,41F).
