carbamazepine toxicity sufficient to require resuscitative
measures may result
Oral contraceptives
concurrent use result in a increased risk of intrahepatic
cholestasis due to decreased metabolism and accumulation of
the contraceptive
Ergot alkaloids
acute ergotism manifested as peripheral ischemia has occurred
Methyl prednisolone
clearance of methylprednisolone is greatly reduced.
Used as a therapeutic advantage to reduce the dose.
Theophyllines
increased theophylline serum levels with toxicity occur
Triazolam
triazolam bioavailability increased, resulting in increased
CNS depression
Cisapride
in vitro indicate that troleandomycin, erythromycin and clarithromycin
markedly inhibit cytochrome p450 IIIA, mainly responsible for
metabolism of cisapride. Concurrent use is contraindicated
Indication:
Streptococcus pnemonia
Adverse Reaction:
Most frequent- Abdominal cramps and discomfort (dose related)
Infrequent- Nausea, vomiting. diarrhea Hypersensitivity- Anaphylaxis, urticaria
Contra-Indications:
Hypersensitivity
Special precautions:
Superinfection-Use of antibiotics (especialy prolonged or repeated therapy) may result in bacterial or fungal overgrowth of non-susceotible organisms. Take appropiate measures if this occurs
Warnings
Hepatic effects- Troleandomycin has been associated with an allergic cholestsatic hepatitis. These have reversed on drug discintinuatiuon.
Readministration reproduces hepatoxicity, often within 24 to 48 hours.
Monitor liver function tests and discontinue if abnormalities develop.
Pregnancy- Safety for use during pregnancy has not been established.
Dosages/ Overdosage Etc:
Indication:
Streptococcus pnemonia
Dosage:
Continue therapy for 10 days when used for streprococcal infections Adults- 250 to 500mg, 4 times a day. Children- 125 to 250mg every 6 hours
Patient Information:
1.Take at evenly spaced intervals during the day, preferably around the clock.
2. Complete full course of therapy.
3. Take until gone

Pharmacology:
Synthetically derived acetylatehyde ester of the macrolide oleandomycin
Pharmacokinetics:
Peak serum levels of 2mcg/ml are attained following a 500 mg dose. serum levels are detected 12 hours later, 20%of the drug is reciverd in the urine. Significant quantities are excreted in the bile.
