Drug interactions- summary-

Lidocaine prolongs duration of action of suxamethonium,

benzodiazepines and barbiturates raise the convulsive threshold to lidocaine

vasopressors potentiates pressor effects of adrenaline

BP may increase with non-selective beta blockers

TCA , halogenated inhalational anaesthetics and alpha blockers general anethestics
may increase sensitivity of myocardium to dyshthmics effects of epinephrine

Lidocaine may decrease levels and effects of codeine, oxycodone, tramadol, carbamazepine,
natacilin, nervirapine, phenobarbital , phenytoin, rifamycin

Local  anesthesia

Lidocaine - the effects are more likely to occur after systemic admininstration rather
than infiltration,  dizziness, muscle twitching,

local anesthesia of mouth/throat impairs swallowing and increase risk of aspiration.
patients cautioned against eating or drinking for 3-4 hours after anesthesia

transient effect of auditory system of neonates, erythema, pigmentation, pain,
local necrosis, pulmonary edema, hyperglycaemia, bradycardia, reduced cardiac
output, anxiety,

Tachycardia, hypertension, cerebral arteroslerosis, ischaemic heart disease ,

Special Precautions

Epilepsy, impaired cardiac conduction, CHF, DM, closed angle glaucoma,
 impaired liver function ( if site of admin is likely to result in high blood levels )
severe renal dysfunction.

Local anesthetic effect may be reduced if injected into an inflamed or infected area.

Cerbovascular insufficiency, hyperthyroidism, neonates, elderly patients in poor
general condition ( optimise patients condition before major block )

Dosage-

Dosage depends on the route .type and extent of surgical procedures

Maximum dose of lidocaine given with epinephrine 7mg/kg and > 500mg mg

Child - 3mth to -12yr- max dose 3mg/kg .
ideal body weight should be used with high body weight
