Drug interactions -summary-

Drugs that should not be coadministered with delavarin mesylate-

Anticonvulsants agents, pheytoin, phenobarbital -
may lead to antivirologic response and possible resistence to delavardine

Antihistamine, astemizole, terfenadine -
contraindicated due to potential for serious or life threatening reactions such as
cardiac arrhythmias

Antimycobacterials, rifabutin, rifampicin-
may lead to loss of virologic rersponse and possible resistence to delavardine

Ergot derivatives, dihydroergotamine, ergonovine, ergotamine, methylergonovine-
contraindicated due to potential for serious or life threatening reactions

GI motilitiy agents - cisapride
contraindicated due to potential for serious or life threatening reactions such as
cardiac arrhythmias

Herbal products- St Johns wort-
may lead to loss of virologic rersponse and possible resistence to delavardine

HNG-CoA inhibitors - lovastatin, simavastatin -
contraindicated due to potential for serious or life threatening reactions

Neuroleptic -pimozide
contraindicated due to potential for serious or life threatening reactions such as
cardiac arrhythmias

Sedative/hynotics, alprozolam, midazolam, trazolam-
contraindicated due to potential for serious or life threatening reactions such as
prolonged or increased sedation, or respiratory depression

HIV infection

Adverse reactions-

Cardiovascular - abnormal cardiac rate and rhythm, cardiac insufficiency, cardiomyopathy
                             hypertension, migraine

CNS-    abnormal cordination, amnesia, change in dreams, confusion, decreased libido,
            disorientation, dizziness, emotional liability

Dermatologic - angioedema, dermatitis, discolored skin, eryhthema multiforme, fungal dermatitis,
                          skin odour, skin nodule

GI -             anorexia, bloody stool, constioation, decreased appetite, diarrhoea, dry mouth,
                   dyspepsia, flatilence, gastrointestinal reflux increased thirst, jaundice

GU -         amenorrhea, breast enlaregement, hematuria, urinary tract infection, impaired
                urination, impotence, kidney pain,

Hematologic/ lymphatic -  brusing, eosinophillia, granulocytosis, purpura, spleen disorder,
                          thrombocytopenia, prolonged prothrombin time

Musculoskeletal -  artlhalgia, arthiritis of single or multiple joints bone disorder

Ophthalmic- blurred vision, conjuntivitis, diplopia, dry eyes

Respiratory -   chest congestion, hicups, largyngitis, rhinitis

Special Precautions-

Skin rash - severe rash including rare cases of eryhthema multiforme and St Johnson -syndrome
have been reported in pay=tients receing delavardine.

Symptomatic relief has been obtained using diphenhydramine hcl , hyroxyzine hcl
or topical corticosteroids

Hepatic function impairment - caution should be excercised when administering delavardine
to patients with impaired hepatic function.

Pregnancy- delavardine should be used in pregnancy only justifies the potential risk to fetus,

Lactation- mothers should be instructed not to breast feed if they are receiving delavardine,

Children- safety and efectiveness of delavardine in combination with delavardine have not
be established in children below 16 years of age. 

Elderly- caution should be eccercised in elderly patients due to ncreased frequency of
decreased frequency of renal, hepatic or cardiac function and of concomittant disease or
other drug therapy

Indication-
HIV infection

Dosage-
400mg ( four 100mg or two 200mg tablets ) 3 times a day.
Delivirdine mesylate should be used in combination with other appropiate antiretroviral
therapy

The 100mg derivarine mesylate tablets may be dispersed in water prior to consumption.
To prepare a dispersion add four 100mg derivarine mesylate tablets to at least 3 ounces
of water, allow to stand for a few minutes and then stir until a uniform dispersion occurs.
The dispersion should be consumed promptly.
The glass should be rinsed with water and rinse swallowed to insure that the entire dose
is consumed,
The 200mg tablets should be taken as intact tablets, because they are not readily dispersible
in water.

1. Patients should be informed that delavardine is not a cure for HIV infections

2. Patients should be adviced that toxicity of delavardine is rash and should be adviced
   promptly to notify their physician should rash occur.

3. Patients should be advicded to take delavardine mesylate every day as prscribed
    and should not alter the dose without consulting their doctor.

4. Patients taking both delavardine and antacids should be adviced to take them at least
    1 hour apart.

4. Because delavardine may interact with certain drugs, patients should be adviced to
    report to their doctor the use any prescription, non-prescription medication or herbal products
    particularly St Johns Wort

Pregnancy-
