Drug interactions- summary-
Droperidol + anesthesia -
certain forms of conduction anesthesia (eg spinal anesthesia, some epidural anesthesia)
can cause peripheral vasodilation and hypotension
CNS depresants ( eg barbiturates, tranquilizers, opioids , general anesthetics) + Droperidol/
Droperidol + CNS depressants ( eg barbiturates, opioids, general anesthetics )-
CNS depressants have additive and potentiating effects with droperidol, thus the droperidol
dose will be less than usual. Reduce dose of other CNS depressants
Epinephrine + Droperidol -
ephinephrine is not recommended as treatment of droperidol - induced hypotension
Parentral anesthetics - eg fentanyl -
hypertension has been reported following coadministration.
Adverse Reaction:
Adverse reactions-
Cases of prolonged QT elongation reported in patients receiving droperidol at doses or below
recommended doses
Droperidol is contraindicated in patients with known or suspected QT elongation
Most common adverse reactions- post operative drowsiness, restlesness, hyperactivity,
and anxiety, chills or shivering, laryngospasm, and brochospasm
Contra-Indications:
Known or suspected QT prolongation
Special Precautions-
Risks for QT prolongation- administer droperidol with caution in presence of risk factors-
- clinically significant bradycardia
- any clinicall significant cardiac disease
- treatment of Class I and Class II arrhyhtmias
- treatment with monoamine oxidase inhibitors ( MAOIs )
- concomitant treatment with other drug products known t o prolong the QT interval
- electrolyte imbalance in particular hypokalemia, hypomagnesemia, or concomittant
treatment with drugs eg diuretics that may cause electrolyte imbalance
Hypotension - keep fluids and other common measures to manage hypotension
Opioids- when required initially use opioids in reduced doses
Neuroleptic malignant syndrome- altered consciousness, muscle rigidity and autonomic
instabilty have occurred inpatients who received droperidol.
Special risk - reduce the initial dose ofdroperidol in elderly , debilated and poor -risk patients
Renal /hepatic function impairment- administer dropridol with caution in patients with liver and
kidney dysfunction
Pregnancy- Use during pregnancy only if the potential benefits justifies the potential risk
to the fetus.
Lactation- Excercise when droperidol is administred to a nursing mother
Children- Safety of droperidol in children less than 2 years of age has not been established
Monitoring - mnitor vital signs and ECG routinely
Dosages/ Overdosage Etc:
Indication-
Antiemetic
Dosage-
Initial dose - 2.5mg IM or slow IV. Additinal 1.25mg dose of droperodol may be administered
to acheive the desired effect.
However administer the additional dose with caution
Children- for children 2 to 12 years of age, maximum recommended dose is 0.1mg /kg taking into
account the patients age and other cliical factors.
Adminster additional dose with caution.
CNS depressant drugs- other CNS depressants drug and general anesthetics have additive
effects on droperidol. Folloing the administeration of droperidol, reduce other CNS depressant
drugs.
Pregnancy and lactation:
Pregnancy-
Use during pregnancy only if the potential benefits justifies the potential risk
to the fetus.
