Indication Type Description:
Drug Interaction:
Pioglitazone - Ketoconazole, inhibits the metabolism of piogilatazone ( in vitro ) Increased hypoglycemic effect with alcohol
Metformin- Additive effect of sulphonylureas Antagonistic effects with diuretics , corticosteroids, phenothiazines, thyroid products, Ostrogens, oral contraceptives, phenytoin, nicotinic acid Sympthomimetics, Ca channel blockers and isoniazid
Pioglitazone admin of a thiazolidinesdione with OC reduces the plasma conc. of the hormones which may result in loss of contraceptive effectivity
Metformin - Lactic acidosis with alcohol and potentiation of hypoglycaemic effect. Cimetidine and furosemide may increase plasma - metformin levels Drugs eliminated via renal tubular secretion may increase metfomin levels
Indication:
Diabetes mellitus Diabetic insipidus
Adverse Reaction:
Pioglitazone - Pharyngitis, odema, headache, upper respiratory tract infection Sinusitis, Anemia, GI disturbances, weight gain, Visual disturbances Dizziness, arthalgia, Haematuria, impotence
Metfomin- Anorexia, nausea, vomiting, diarrhoea. Weight loss, flatulence, Occasional metallic taste, Weakness, Hypoglycemia, Rash, malabsorption of Vitamin B12, lactic acidosis in presence of renal failure and alcoholism
Contra-Indications:
Hypersensitivity Diabetic coma, Diabetic ketoacidosis Severe renal or hepatic impairment Recent MI, CHF, type diabetes mellius Severe infection, Acute or chronic metabolic acidosis with or without coma Stress, trauma, Severe impairment of yhtroid function Dehydration, Acute or chronic alcoholism Child < 18 years Pregnancy Lactation
Special Precautions-
Elderly- Monitor renal function regularly Patients receiving pioglitazone in combination with insulin or oral hypoglycemic agents may be at risk of hypoglycemia Odema, congestive heart failure, Hepatic dysfunction Jaundic Premenopausal anovulatory women, Anemia Monitor liver function

Dosage-
Monotherapy- Usual starting dose - piogilatazone/metformin 15mg/ 500mg or 15mg/850mg 1-2 times daily adjust dose gradually based on therapeutic response
