Indication:
Scizophrennia and psychoses
Adverse Reaction:
Side effects-
More sedating hypotension common when treatment initiated
Respiratory depression
Contra-Indications:
Cautions-
Should be used with caution in patients with cardiovascular disease,
Antipsychotic drugs may be contraindicated in comatose status,
CNS depression and phaechromocytoma
Driving-
Drowsiness may affect performance of skilled tasks
( driving and operating machinery), especially at start of treament ,
effects of alcohol enhanced
Withdrawal-
There is high risk of relapse if medication is stopped after 1-2 years.
Withdrawal should be gradual after long term therapy, should be
gradual and closely monitored to avoid risks of acute withdrawal synptoms
Hepatic impairment-
Can precipitate coma if used in hepatic impairment
Renal impairment-
start with small doses in severe impairment becuase of cerebral sensitivity
Dosages/ Overdosage Etc:
Indication-
Scizophrennia and psychoses
Dosage-
Initially 75mg daily in divided doses increased at weekly intervals
by steps of 25mg according to response
Usual max 300mg daily
Elderly- initially 15-30mg daily,
Child- and infant over 1 year
Schizophrenia or behavoiur disorders only
Initially micrograms daily for 10kg child,
increased according to response but maintenance should not
exceed twice initial dose
Short term adjuntive management of severe anxiety,psychomotor agitation
and violent impulsive behavour
Initially 15-30mg (elderly 5-10mg) daily in divided into 2 doses,
taking the larger dose at bed time , adjusted according to response
Child- not recommended
Pregnancy and lactation:
Pregnancy-
Following maternal use during third tromester , neonatrs shoould be
monitored for symptoms including agitation, hypertonia, tremor,
drowsiness,feeding problems and respiratory distress
Breast-feeding-
Limited information avial for short and long effects while breast feeding
