Hypotension, tachycardia, flushing, headache, palpitation, sycope, confusion, nausea, vomiting, abdominal pain. Apprehension, restlessness, weakness and vertigo. False decrease in serum cholestrol.

Sublingual tabs: Localised burning,delayed dissolution in dry mouth.

Ointment: Contact dermatitis.

Acute MI when shock present. Marked anaemia, Cerebral haemorrhage, hypertension. Obstructive hypertrophic cardiomyopathy, low cardiac out put,sceondary to hypovalaemia, inferior M.I. with right ventricular invovement , raised intracranial pressure, cardiac tamponade.

Special precautions: Mitral valve prolapse,arterial hypoxaemia,glaucoma,elderly,hypothyroidism,malnutrition, pregnancy,lactation.

Pharmacology:

Iosorbide dinitrate has a direct action on the vascular smooth muscle producing vasodilation of the arteries and veins. Vasodilation profoundly reduces cardiac preload while the arterial vasodilation reduces after the afterload.; it lowers the systolic BP & systemic vascular resistence and reduces intracardiac dimensions.
