Interacting drugs - summary
             
   + Valproic acid
               
   Charcoal   
      valproic acid absorption decreased,  reduce effectiveness or toxicity
   
    Chlorpromazine
     decreased clearance and increased half-life of valproic acid
     
    Cimetidine
      decreased clearance and increased half-life of valproic acid
 
     Felbamate
       valproic levels increased , possibly producing toxicity
 
     Rifampicin
       rifampicin, increased oral clearance of  valproate by 40%
     
     Salicylates 
       Aspirin elevate valproic acid levels, leading to toxicity
              
        Valproic acid +
 
        CNS depressants/  Alcohol  / Benzodiapines    
        CNS depressant activity may be potentiated
         pharmacokinetic parameters of benzodiapines increased,enhanced 
          sedation
         
         Valproic acid + Carbamazepine  or  Carbamazepine + Valproic acid
           variable changes in carbamazepine concentrations with decreased
           valproic levels and possible loss of seizure control
                   
           Valproic acid +  Clonazepam  or   Clonazepam +  Valproic acid 
            increased toxicity (drowsiness ) of clonazepam,  reduced  
             efficacy of both drugs
                                
           Clozapine 
           Clozapine concentrations slightly elevated
           
             Ethosuximide
              Ethosuximide blood levels may increase or decrease 
           
              Valproic acid +  Lamotrigine or  Lamotrigine + Valproic acid
                Valproic acid serum levels may be decreased while
                 lamotrigine levels may be increased
                     
               Valproic acid + Phenytoin or Phenytoin + Valproic acid 
                increased action of phenytoin even at therapeutic levels or increased                                     metabolism  of valproic acid with pharmacologic effects may occur       
                   
                Phenobarbital /  Primidone 
                 concurrent use may result in increased phenobarbital levels and  
                  an increase in CNS  effects
             
               Warfarin
                 potential exists for valproate to displace warfarin from protein binding
                   sites. Monitor coagulation tests
                
                Zidovudine 
                 Zidovudine AUC may be increased
